Donate your time or services.
Canadian Donations
The KDA is pleased to announce
that our Canadian friends can now
make donations directly to a
Canadian laboratory that is
conducting Kennedy's Disease
research. These donations are
considered tax deductible for
Canadians.
Donations can be made through the
SMBD-Jewish General Hospital's
website at
www.jgh.ca or by
mail. Click on the link to
"Supporting the JGH Foundation"
and then "How to Donate" You will
have a choice of making a donation
on-line or by mail. If the on-line
donation form is used, select on
of the following fund categories
"In Honour", "In Memoriam" or
"Lady Davis Institute for Medical
Research". Then, in the message
box, type in "The gift is for
Kennedy Disease Research
(GL#4000-5441)".
If you use the mail, you will need
to use the attached form. Clearly
indicate on the form in the
area "Please designate this gift
to" that this donation is
for "Kennedy Disease Research
(GL#4000-5441)". Mail the form
along with your check to:
